• To gain access to the Medical Group's Data Mart, you must first complete a 2-day Cognos training.
• To register for the training, please visit training and classes and fill in your details.
• Once you have registered, you will receive an email containing the Medical Group's Data Mart User Account/HIPAA form. You need to complete this form, obtain your supervisor's signature/approval and then send it to Maria Lourdes Imasa via email. 
• If you are a new user, you must attend the first day's morning session.
• After you have completed the Cognos training, you will be granted access to the Data Mart.
• Please note that only individuals with a UCSF email address, i.e., an email address that ends with "@ucsf.edu," are eligible for this training.

The training classes will cover the basics of the Faculty Practice Organization DART tools and provide you with an overview of the available reporting content. This content includes specific types of data or reports that can be used for particular purposes or benefits.
